Built by someone who got tired of watching the system fail people.

Code 1 didn't start as a business idea. It started as a breaking point. After more than 15 years in emergency medicine, Dr. Cashio had seen the same pattern repeat itself enough times that he couldn't keep showing up to the last stop of a journey that should have gone differently upstream.

§ 02 · The Origin

The ER is where you end up when nobody caught it earlier, leading to catastrophic heart failure.

Dr. Cashio spent over a decade working emergency medicine and critical care. He was good at it. The ER demands a specific kind of physician: calm under pressure, fast with decisions, capable of holding complexity without flinching. He was all of those things.

But the ER also gave him a front-row seat to a pattern that never stopped being frustrating. High blood pressure that had been building for years. Blood sugar that had been trending the wrong way for a decade, leading to a critical overload. Weight gain, metabolic dysfunction, hormonal decline, cancers that went undiagnosed. Problems that had been developing slowly, quietly, in people who technically had a primary care physician but couldn't get to them, and when they did, got 7 minutes and a prescription.

They weren't being neglected by bad doctors. They were being failed by a system that rewards volume over depth and efficiency over actually knowing the patient.

Dr. Cashio got tired of being the last stop.

So he walked away from emergency medicine and built something designed to be the first real stop. The place that catches things early, knows the full picture, and builds a plan that works before the situation becomes a crisis. That's Code 1.

§ 03 · The Philosophy

What Code 1 stands for. And what it stands against.

Code 1 is not anti-medicine. It's not anti-doctor. Physicians working inside conventional systems are doing the best they can inside a structure that doesn't give them the time or the resources to practice the way they were trained to.

Code 1 is anti-lazy-thinking. There's a meaningful difference.

"Your labs are normal" is not a complete answer when the patient clearly doesn't feel normal. "You're just getting older" is not a diagnosis. "Just watch it" is not a plan. Vague reassurance in place of actual interpretation, testing without explanation, medication without context, fragmented care where nobody owns the full picture… these are the things Code 1 was built to replace.

The standard here is simple: know the person, tell them the truth, build a real plan, and stay in it with them. Everything else follows from that.

§ 04 · Dr. Cashio, MD MS

The physician behind the practice.

Dr. Cashio grew up in Texas and Southern California, the product of a Cajun family and a state that takes its football seriously. He went to UT Austin for his undergraduate degree, UT Houston and MD Anderson for graduate school in developmental biology and genetics, and UT Health Science Center at San Antonio for medical school. His residency was at the University of Virginia in Charlottesville.

Before medicine, he served in the United States Marine Corps as an Intelligence Analyst and lived in Okinawa, Japan for two years. That experience left a mark. The Marine Corps standard on discipline, accountability, and doing the hard thing correctly doesn't leave you, and it shows up in how Dr. Cashio practices medicine.

He brings a particular kind of directness to patient care. Not harsh. Direct. There's a difference. He'll tell you what your labs actually mean, what the risk actually is, and what you actually need to do about it. He won't waste your time with vague reassurance and he won't tell you what you want to hear if it's not what's true.

Outside the clinic, Dr. Cashio lifts. He is learning to play the guitar (finally). He's raising two boys. He plays first-person shooters, follows Japanese culture with genuine interest, and cares deeply about taking care of veterans.

Matt has spent his career on the front lines of medicine — from stabilizing trauma patients in EDs to providing care in Iraq and Afghanistan, rappelling out of helicopters, and training the next generation of military clinicians. Doctorate of Science in Emergency Medicine PA Studies from Baylor. Now based in Harker Heights, he works directly with patients to understand their health, build personalized plans, and make informed decisions without the overwhelm.

Off-hours: lifting heavy things, hiking, or playing Call of Duty zombies with his son.

Critical care nurse with over 10 years of experience. Rebecca brings the kind of calm, focused energy that makes patients feel like they're in exactly the right hands. Around here, we call her "The Quiet Professional." BSN from University of Mary Hardin Baylor.

Currently a fixture at powerlifting gyms around Austin. Previous career as a circus tightrope walker remains unconfirmed.

Fourteen years across the ER, Pediatric ICU, and outpatient surgery. Whatever walks through the door, Jenna has probably seen it, handled it, and kept her composure doing it. Born and raised in Austin, now living in Dripping Springs.

Off-hours: lifting weights, chasing after her family, or somehow doing both at the same time.

If you've ever called, emailed, or had any question answered before you even finished asking it, that was probably Sandra. 35 years in the insurance industry as a claims adjuster and manager before landing at Code 1. She's the person who keeps the details from falling through the cracks.

What To Expect

Bring your ID and insurance information to your first visit (this makes referrals easier). Parking is freely available right next to the clinic. You do not need to show up early — appointments start on time. Please let us know if you're running late. We have coffee, water and drinks, and a relaxing atmosphere. We hope you'll find this the easiest doctor's appointment you've ever had.
